How to Activate Medical Marijuana Registry Account and Card

This article outlines how a patient or caregiver can access the Medical Marijuana Registry (MMR) and activate their MMR account and card. Patient and caregiver accounts are created by the physician providing them a medical marijuana recommendation.

Access Email

1. After a patient or caregiver's physician has created their MMR account, an email will be sent to the patient or caregiver (Example below).
   
   

   This message is automatically generated based upon the recent activity regarding your Ohio Medical Marijuana Control Program Patient/Caregiver Registration.

   Please do not reply to this email.

   Dear FirstName LastName,

   This email provides notice of the creation of an account in the Ohio Medical Marijuana Patient & Caregiver Registry. To complete your registration, click here and follow the instructions. You must complete your registration within ninety (90) days of the date of your recommendation. If you fail to do so, your recommendation will be voided, and you will need to obtain a new recommendation from your physician prior to registration.
   
   Once your registration is complete, your medical marijuana card will appear on your Registry profile. You will not receive a copy of your registration card by mail. If you would like a physical copy of your registration card, you must click the "Download" button below your card and print it out. Otherwise, you should maintain an electronic version on your mobile device.
   
   For additional information regarding Ohio's Medical Marijuana Control Program and to find answers to frequently asked questions, please visit www.com.ohio.gov/mmcppatients. If you are registering with indigent or veteran status, additional documentation is required for that status to appear on your registry profile. 2. Access the email inbox of the email address that you provided to your physician and locate the Ohio Medical Marijuana Registry User Activation email.
   • If the email is not in your inbox, please be sure to check your spam and junk folders.
   • If you are unable to locate the email after checking other folders, please reach out to the MMR Administrator by sending an email to [e-mail].
3. Click on the link within the text "click here and follow the instructions".
   • If your activation link has expired, please reach out to the MMR administrator at the email provided in step 2.
4. Clicking on the link will direct you to a page where you can setup a password for your MMR account.

Create Account Password

1. When you create your password, you must follow these requirements:
   • Minimum of eight characters
   • At least one upper care letter
   • At least one lower case letter
   • At least one special character (E.g. @ # $ % ^ & * )
   • Maximum of 255 characters
2. Input your desired password in both the "Password" and "Password Confirmation" fields.
3. Click "Save Password".
4. You will be directed to the OH MMR.

Activate MMR Card

1. Input your email address in the "Email" field and enter your password in the "Password" field.
2. Click "Sign In".
3. You will be directed to the User Preferences page.
4. Select your preferred timezone from the drop down menu.
5. After you select your timezone, click "save Preferences"
6. You will be directed to the OH MMR home page.
7. In the "Registry Details" section, you will find a "Card" tab. Click "ACTIVATE CARD".
   
8. You will be directed to the Legal Statements page.
9. Read the legal statements.
   • If you have any questions regarding the legal statements, please contact your physician.
10. Once you have read the legal statements and you are ready to proceed, click "ACCEPT"
    
11. You will be directed to the Summary page. On this page you will see your profile details.
12. Click "Continue" to complete your activation.
    
13. Once “Continue” is selected, The page will display a “ACTIVATED: Registry Card has been activated successfully” message.
    
14. Once the activation is complete, your Medical Marijuana Registry card is activated and will be displayed on the OH Medical Marijuana Registry site's "Registry Details" section and the "Card" tab.
